Physical Therapist - Home Health
Location: Elizabeth, West Virginia
Counties Covered: Wood, Wirt, Calhoun
Job Details
Setting: Home health, traveling to patients' homes
Schedule: Monday to Friday, 8:00 AM – 4:30 PM (40 hours/week)
Patients per day: Approximately 5
Patient Types: General orthopedic (hips, knees), generalized weakness, strengthening, cardiac rehab
EMR System: EPIC
Orientation: Training on EPIC and with another Physical Therapist
Uniform: Navy top, tan bottom
Weekend/Holiday Requirement: None
Experience & Skills
General Physical Therapist duties in home health setting
Positive attitude toward learning and using EPIC system
Ability to provide excellent patient care in a home environment
Additional Information
Rehab support includes 2 Physical Therapist Assistants (PTAs)
No 24/7 pharmacy support needed
Contract length: 13 weeks
Requirements
Required Qualifications
Licensed Physical Therapist authorized to practice in West Virginia
BLS (Basic Life Support) Certification
Valid driver’s license (required for travel between counties)
