Full Job Description

We are seeking a dedicated and compassionate Nurse Case Manager (RN) to join our growing home care team. This full-time Nurse Case Manager (RN) position focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in the Ashburn and Northern Virginia communities. The ideal RN is someone who has a passion for nursing, strong clinical, and great customer service skills. Join the best and brightest in the industry and deliver personal and powerful care.

Schedule:
Tuesday through Saturday
Shift options: 10:00 AM – 6:00 PM or 11:00 AM – 7:00 PM

Key Responsibilities:

• Conduct initial client assessments and routine reassessments in home settings

• Conduct hospital discharge assessments

• Develop and update individualized care plans

• Collaborate with caregivers, families, and interdisciplinary teams

• Monitor client progress and ensure compliance with care standards

• Maintain accurate and timely documentation

• Completing annual skills assessments and competencies for LPNs and CNAs/Caregivers,

• Training and supervising healthcare personnel: LPNs, CNAs, and Caregivers.

• Attend dischange planning as needed

Qualifications:

• Active Virginia RN license in good standing (Compact/Multi-state)

• At least two years of clinical experience (home care, case management, or patient assessment)

• Strong clinical judgment and communication skills

• Ability to work independently and manage a flexible schedule

• Current CPR Certification, AHA BLS
